1. Once you finish enrollment, our office will contact you within 24 business hours to schedule your initial appointment.
2. At the end of the enrollment you will be able to add family members.
3. You will need to have your payment method ready in order to complete your enrollment. Please call the office at 937-557-5657 if you would like to make other payment arrangements.
4. If you are signing on with your employer as a sponsor, be sure to list them.
5. Please note we need at least 48 hours to schedule a new patient in order for our team to review your chart.
